Please note that we are a Self-Pay Practice as we are not in-network with any commercial insurance. We are happy to bill your Traditional Medicare if it is your primary.

New Patient
Intake Forms
We make getting started simple with secure digital forms. Before you visit, we'll send you a link to complete your paperwork online at your convenience. Once submitted, your information is securely sent directly to our office and automatically added to your chart — saving time, reducing paperwork and helping your appointment run smoothly
Consultation
During your visit, you'll have a same-day one-on-one consultation with Dr. Braudt to review your health history, discuss the reason for your visit, and go over your personalized care plan. This dedicated time allows us to fully understand your needs and ensure your care is tailored specifically to you.
X-Rays & Therapies
Depending on your individual needs and care plan, additional therapies or on-site digital x-rays may be recommended. These services are optional and only used when Dr. Braudt determines they would be beneficial to your care.
